The My Health preventive program was launched on May 5. It can be used by people aged 20 and older, the basis for qualification is the answers provided in the survey, and the results of the ordered tests are used by the primary healthcare facility to establish an individual health plan for the patient. It is worth taking advantage of the possibility of comprehensive tests.

Moje Zdrowie is the largest ever, first population-based health prevention program in Poland. It is available to people over 20 years of age. The program aims to build a healthy and conscious society.

My Health is an individual health plan tailored to the patient's needs. The program encourages self-care and reminds how important regular check-ups are. It is an adult's health assessment, thanks to which health threats will be detected. Quick signaling of a problem will help direct the person to the right diagnostics.

The program allows for early detection of lifestyle and infectious diseases, which gives a chance for more effective treatment. It also strengthens the patient's relationship with primary health care and restores the role of primary health care as a place that not only treats, but above all takes care of health on a daily basis. At the same time, it does not exclude the possibility of using daily primary health care, preventive tests, referrals for tests as part of an individual medical care plan (IPOM).

To participate in the program, all you need to do is fill out a short survey, and you don't have to leave your home. You can do it conveniently and quickly via the Patient's Internet Account or use the mojeIKP mobile application. You can also go to a clinic, where a medical employee will help you fill it out. Based on the survey, the patient receives a set of tests tailored to their age, gender, and lifestyle.

The medical staff will then prepare an Individual Health Plan tailored to the patient's needs. During the summary visit, the doctor, nurse or midwife will discuss the results, risk factors and further recommendations: lifestyle, diet, physical activity, vaccinations and prevention. The scope of this visit depends on the results of the tests and the patient's age. The patient can also benefit from educational advice based on their needs. Patients aged 60+ may be referred for screening for dementia.

All data is transferred to the IKP and, with the patient’s consent, is available to doctors.

The foundation for the implementation of the program is primary health care facilities. As part of the preparations for the implementation of the program for primary health care employees, training courses, webinars were conducted, and educational leaflets were prepared.

The My Health program is financed by the National Health Fund (NFZ). This year, PLN 193 million has been secured for this purpose. A necessary condition for settling the balance is to perform a basic package of tests and conduct a summary visit with the patient.

POZ will receive:

* PLN 325 for a patient aged 20-59,

* PLN 349 for a patient aged 60 and over.

Clinics will also receive additional funds for educational advice and some research.
